HCFA stands for Health Care
Financing Administration. The Health Care Financing
Administration officially changed its name to the Centers for
Medicare and Medicaid Services or CMS on July 1st, 2001.
The Centers for Medicare & Medicaid Services (CMS) is
a Federal agency within the U.S. Department of Health and Human
Services. The HCFA-1500 form has been
around for many years, and is used by many various health
practitioners and insurance companies to expedite the process of
insurance claims and other billing formalities. Since this form
was developed by a federal agency it is considered "public
domain". Many practitioners pay for bulk stacks of these forms.
