HCFA-1500 Form Help
This form is filled out just like any other
form. Simply click in the appropriate field, or use the tab key
to fill out the area you want.
We suggest that you have the appropriate
training to fill out the HCFA-1500 form. You may have had a class or two
related to insurance billing, but you should also have some good
literature on hand to assist with insurance claim billing as
well. MassageBiz.Net does not provide training or instructions
for insurance claim billing, but we can provide some useful
links to point you in the right direction.

You can also simply print out the HCFA-1500 form and fill it out
manually, but please don't mass photocopy the forms. Instead, tell others to
visit the website to use these forms. Thanks!
We also suggest that you do not navigate away from the page
while creating a HCFA form because you may lose your existing data. The forms
should open in a new window so you should not loose any work unless you close
the new window completely.
Print Settings
Our 1500 form has
been optimized for use with Internet Explorer 6 and higher. To insure that
your forms print properly the margins must be set appropriately.
When you have a forms page open
in Internet Explorer and your ready to print:
- Click on the File
menu.
- Click Print Preview.
- Click on the Page Setup
icon or press Alt.+ U.
- The Page Setup dialog box
will appear. Use these settings: Size=8.5x11in.
Orientation=Portrait. Margins (inches)= Left:.25, Right:.25,
Top:.1, Bottom:.5.
Any other settings may cause
the pages to be printed undesirably. The HCFA form should only
print out with two pages. Pre-printed HCFA's have page two on
the backside of the form. Therefore, you should always include page
